Register as a patient
Step 1
Check that you live inside our practice area, against the following map here. Unfortunately we are unable to register you as a patient if you live outside our practice area.
Step 2
Please print out and complete the forms below:
- Family doctor services registration (permanent patients only)
- Family doctor services registration (temporary patients only)
- New patient registration form - Word | PDF
If you are a temporary patient please complete the temporary registration form not the permanent form. If you cannot print out the forms then you can obtain copies from the Surgery.
Step 3
Please come to reception at the practice to register. When you arrive you will need to bring the following documentation with you;
- Completed registration forms
- Proof of address such as a utility or council tax bill (we cannot accept mobile phone bills)
- An acceptable photo ID such as a passport or driving licence.
Please show the completed forms to reception. Your blood pressure will then be taken and you may be asked to give a urine specimen. When you have completed your registration at the surgery we will automatically issue you with a username and password for our internet booking system. These will be sent to you in the post. You will not be able to make an appointment until you have finished the registration at the surgery
