New Patient Registration
We will be happy to register anyone who lives or works in Islington
In order to register you will need to complete a
New Patient Registration Form (GMS1)

Please ensure you have completed the form correctly in order for us to register you within 72 hours.
Should you have any queries or questions on completing the form you can contact the surgery on 0207 359 0445 and press the option for the query line or alternatively you can send an email to

and put [registration query] in the subject line.

When you visit the practice for the first time either to see a doctor or any other health professional, the reception staff will ask you to do your blood pressure, height and weight. This information will go onto your records.

(e.g. heart conditions, diabetes, Cholesterol, Blood pressure medication) then we would like to invite you for a
FREE NHS Health Check (40-74y)
An
NHS Health Check
will offer guidance on how to prevent developing a chronic long-term condition such as:
heart disease
stroke
diabetes
dementia
kidney disease
In order to book the health check please phone the surgery to make an appointment with
any of our nurses or healthcare assistants 2 weeks after you had a your blood test done
as you will need to have
blood test first
before the initial appointment.
