Online Registration

To register online, please click on the Register Here below to open the form. When you have completed all of the details, click on the “Send” button to mail your form to us. When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

The medical questionnaire is because it can take a considerable time for us to receive your medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

In Person Registration

Alternatively, you may print off a registration form and medical questionnaire, fill it out and bring it in with you on your first visit to the practice or email [email protected]

When you visit the practice for the first time either to see a doctor or any other health professional, the reception staff will ask you to do your blood pressure, height and weight. This information will go onto your records.

Please ensure you have completed the form correctly in order for us to register you within 72 hours.

Free NHS Health Check

If you are aged between 40 and 74 with No pre-existing medical conditions (e.g. heart conditions, diabetes, Cholesterol, Blood pressure medication) then we would like to invite you for a FREE NHS Health Check (40-74y)

An NHS Health Check will offer guidance on how to prevent developing a chronic long-term condition such as: 

  • Heart disease
  • Stroke
  • Diabetes
  • Dementia
  • Kidney disease

In order to book the health check please phone the surgery to make an appointment with any of our nurses or healthcare assistants 2 weeks after you had a your blood test done as you will need to have blood test first before the initial appointment.


Free Hepatitis B or C Blood test

For your free blood test ask the receptionist to book an appointment with the nurse.
